It is important that the information you provide is accurate and that you disclose all relevant facts as this could affect the opinion you are given.

Providing information about others.

Before you provide information about any person other than yourself, you must ensure that they understand how their information will be used (see our Privacy Policy for details) and that you have their authority to provide it to us and to give the relevant consents on their behalf. In most cases you should expect to receive a response within 5-7days. However, we are dependent on Dr Nick’s availability and third party computer and communications systems which may affect response times.
