INSOMNIA
Insomnia is a condition of unsatisfactory sleep. This may be in terms of difficulty getting to sleep, staying asleep or waking too early. This may be associated with daytime fatigue, irritability and mood disturbances.
Normal sleep requirements vary considerably. Some people function well on 4 hours per night while others need 8-10 hours. Sleep requirements also tend to decrease with age. One third of adults in Western countries experience difficulty with sleep at least once a week, it is also twice as common in women. It can be a chronic condition and can last for years. CAUSES

There are several medical conditions such as Restless Legs, Parkinsons disease, heart problems, arthritis and menopause which often are complicated by insomnia.
Psychological problems such as depression, anxiety, stress, bereavement and alcohol or drug abuse often cause insomnia. Some antidepressants especially SSRI drugs can impair sleep.

TREATMENT
Sleeping tablets are NOT the answer. 10 million prescriptions every year are dispensed for sleeping tablets. These drugs should only be prescribed for short term use, one week or less and should be avoided in the elderly.

SLEEP HYGENE
This advice will help up to 30% of cases. Dont drink caffeine in the evening and avoid alcohol and cigarettes. Dont sleep during the daytime. Do regular exercise but not late in the evening. Dont use computers for games before bed as this can overstimulate the brain. Use your bedroom only for sleep.

DRUGS
Short acting benzodiazepines or Z drugs are very effective for short term use only but they carry a serious risk of dependence if used for more than two weeks. Melatonin can be useful especially in the elderly. Some antidepressants, especially the tricyciclics such as amitriptylline can be effective in small doses. Sedating antihistamines are often used but they often cause a hangover effect in the morning. The elderly tend to get more problems with drugs, they can cause confusion and falls leading to serious injury.
